Key Request Form

  • The three-part Key Request form has been replaced by a fillable PDF, making it a faster and more convenient process for departments to request keys. Download now »
    • If you are having issues viewing the online application in your browser:
      • Make sure your Adobe Reader is updated.
      • Depending on your browser, click the small "PDF" icon on the top right of the webpage. Choose to open in Adobe Reader. Follow the prompts. When the PDF opens, click "save a copy."
      • You can also try downloading the form in a different web browser. Click on the link above, and when the webpage appears, choose to download the document (even if it's not appearing within the browser). Open it from where your computer stores downloaded documents and complete the form.
      • NOTE: Mac users, visit this webpage for instructions on how to digitally sign a form.
         
  • Each department has a Designated Signing Authority who will complete the form and manage the process for requests. Please ask your office administrator if you do not know who your designated signing authority is. 
  • Access Control's turnaround time is three days for all correctly submitted key requests.

Reminders for Key Holders

  • All keys are property of the Auraria Campus Facilities Management. You are responsible for its security and use.
  • This key must be returned to Auraria Campus Facilities Management, not your department.
  • If a key is lost, you must notify Facilities Services (303-556-3260) and the Auraria Campus Police Department (303-556-5000) immediately. A nonrefundable fee of $25 is charged for the replacement of a lost key.
  • Return broken key pieces for replacement.
  • This key may not be lawfully copied, except by Access Control in the Facilities Services office.
  • You may not lend or give this key to anyone else.
  • Students must provide a $25 deposit for each key issued, which will be refunded when keys and receipts are returned.
  • Keep all receipts for proper credit.
  • Key numbers and copy numbers must match the approved form.
  • When returning one key and picking up another, credit for the original deposit will cover the new deposit, but you will need to provide the original receipt.
